Renewal for Category A Pharmacists

As per Pharmacy Act 1948 under section 34, all Pharmacist should renew their registration before 31st of December every year. The entire process of registration renewal could be completed from the website by paying the stipulated fees and submitting the required documents. The renewal of registration could be done by click on “Renewal”, in case the registration is due for renewal, it would be highlighted. Just click and follow the instructions.

 Scan Copy of Original Pharmacist Registration Certificate Both Side (Colored) .

 Scan Copy of Last Renewal Certificate (Colored).

 Two Recent Passport Size Photograph. (5 x 4 cms).

 Aadhar Card / V.I. Card /Passport (both Side).

 Order Copy of Release Letter and Appointment Letter or I.D Proof for Govt./ Govt. Undertaking and Private Organization Scan Copy..

 Two days Up-Gradation Training Money Receipt/Certificate.

 Download Address Change Form in case of Address Change in I.D Proof (Aadhaar Card / V.I. Card / Passport).

 Download Surname Change Form in case female married candidate/ others .In this case the candidate first have to change his/her surname physically at Council office before Renewal.

During receiving of Original Renewal Certificate the candidate have to bring all original documents including Affidavit’s.


Restoration of Name

A Registered Pharmacist who fails to renew his/her registration before 31st March of the concerned year, the name of such pharmacist is removed from the Register of Pharmacists in accordance to the provisions of the Pharmacy Act, 1948 and the Rules there under. If the pharmacist desires to re-enter his/her name in the Register, he/she has to apply online and upload following documents and prescribed fees.

 Payment of Fees for Surname change.

Now, after uploading the downloaded form and an affidavit from 1st Class Judicial Magistrate the Applicant has to submit Original Registration Certificate alongwith the Money receipt paid for the purpose within 7 working days to the office of the Council.
